Provides nutrition counseling to Patients for weight Management, eating disorders, digestive diseases, diabetes, and other nutritional interventions related to various disease management programs. Provides support and makes recommendations to clinical staff regarding nutritional management as it relates to patients care. Assists with patient and staff education. When assessing the patient, the age of the patient is taken into consideration.
Will direct and perform duties to ensure that patient has a pleasant and satisfactory experience in the manner in which their healthcare is delivered, managed, and coordinated with internal and external entities while encouraging the patient's participation in decision making. This position requires total support of the PCMH principle, policies, and procedures. Performs other duties as assigned.
Patient relationship: Will greet and welcome the patient in a pleasant, respectful, culturally and linguistically appropriate manner. Assure all patient questions are answered in a positive and cordial manner. Assures the patient has a clear understanding of all communications. Communicates patient’s problem/complaint to the provider. Reinforces to the patients the importance of keeping appointments or communicating changes in their appointments. Asks the required questions for a proper assessment.
Management of patients care: Provides nutritional counseling to patients with high blood pressure, hyperlipidemia, diabetes, obesity, underweight, food allergies, intolerances, food and drug interactions, and eating disorders. PCMH 2
Provides a nutritional assessment on every patient seen. Provides service to the community (i.e. nutrition presentation) and participates in health fairs as requested. Teaches patients with diabetes how to manage their diabetes. PCMH 4A
Sets appointments and follow-up appointments for patient needing nutritional or diabetes education. May do telephonic follow ups. Provides in services to clinical staff as requested. Participates and contributes to Collaborative efforts as requested. Prepares detailed reports regarding patient contacts and instructions given. Follows the Gateway plan for DSME patient education for diabetic patients PCMH 4B. Use patient self-management tools PCMH 4A.
Manages patient information: Documents all patient teaching in a complete and timely manner in the EMR. May need to prepare tracking reports for specific patient population. Assures the patient information and records are completed timely and are current. Adheres to all HIPAA regulations in and outside of the clinic. Communicates patient information as requested to enhance patient services.
Team communication: Approaches patient care as a team involving clinic and non-clinical staff (eg, physicians, mid-levels, educators, appointment staff) to achieve patient care objective. Attends morning huddles if available (PCMH 1G). Position may require supervision.
Demonstration of safe and professional conduct: Submits required documentation in a timely manner (credentialing requirements, license renewals, certifications, CE attendance, etc.)
Registered Dietitian (ADA), Licensed dietitian (Texas State Board) plus 2 years of experience in nutrition counseling and diabetes education. Certified or eligible for certification in Diabetes Management or Weight Management. Experience in working with the public. Must be computer literate. Must be current BLS/CPR certified. Bilingual (English/Spanish) required.
